Management of Pain

The approach to pain management should begin with a complete assessment of the current pain complaint and commonly associated problems such as insomnia and psychological distress, and a thorough pain management history. Depending on the nature of the pain complaint, treatment may involve pharmacologic or nonpharmacologic approaches, or both. A large number of analgesic medications are effective for treating pain ranging from mild to severe intensity. Product selection must be guided by both the efficacy of the analgesic and its adverse event profile. The weakest effective analgesic at the lowest effective dosage should be selected to minimize risks of adverse events.

Analgesic agents attack pain transmission at several different levels of pain pathways. Mild to moderate pain often is managed with simple analgesics such as aspirin, acetaminophen, nonsteroidal anti-inflammatory drugs (NSAIDs), or cyclooxygenase-2 (COX-2) inhibitors. Opioids remain the mainstay of therapy for moderate to severe pain, particularly when pain has a major central component.
