Cholesterol: what is it?

A complex molecule that has a core structure similar to that of a steroid molecule; it is attached to a fatty acid to form cholesterol ester. Cholesterol is vital to life, being an essential component of all animal cell membranes (plants do not contain cholesterol), and is required to make bile acids (needed for fat absorption), adrenal hormones corticosteroids, sex hormones (androgens, oestrogens), and vitamin D.

Cholesterol forms part of the diet and is also synthesized in the body, mainly in the liver. It is carried in the blood by lipoproteins. Too much cholesterol in the bloodstream (hypercholesterolaemia) can lead to deposits (plaques) on the inner wall of the arteries, giving rise to atherosclerosis. In the advanced stage, usually when a vessel is more than 50% blocked, atherosclerosis can result in thrombosis, causing a stroke or heart attack.

A high circulating concentration of low-density lipoprotein (LDL) cholesterol is one of the most important risk factors for developing coronary heart disease. It enhances the adverse effects of other risk factors, such as smoking, obesity, hypertension, and diabetes.

Reducing circulating cholesterol lowers the incidence of heart attacks and other events related to coronary artery disease. Cholesterol reduction is achieved by dietary modification (reducing the intake of saturated fats and increasing consumption of fibre, oats, and pulses), which is the usual first-line treatment, or by lipid-lowering drugs.
