Ultram
Ultram contains the active ingredient tramadol hydrochloride, which is a type of medicine called an opioid painkiller.
Opioid painkillers work by mimicking the action of naturally occurring pain-reducing chemicals called endorphins.
Endorphins are found in the brain and spinal cord and reduce pain by combining with opioid receptors.
Tramadol mimicks the action of our natural endorphins by combining with the opioid receptors in the brain and spinal cord.
This blocks the transmission of pain signals sent by the nerves to the brain.
Therefore, even though the cause of the pain may remain, less pain is actually felt.
Tramadol also works by enhancing the activity of neurotransmitters in the brain and spinal cord.
Neurotransmitters are chemical compounds that act as chemical messengers between the nerve cells.
Ultram enhances the effect of the neurotransmitters serotonin and noradrenaline, and this action also helps relieve pain.
Tramadol is a fairly strong painkiller that is used in the management of moderate to severe pain.
Ultram capsules are taken by mouth up to six times a day, depending on the severity of the pain.
This form of Ultram should not be taken more often than every four hours and you should not exceed the dose prescribed by your doctor.
For more prolonged pain relief, modified release forms of Ultram that can be taken once or twice a day are available.

Dosage
Adults
The usual starting dose for chronic pain is 25 mg once a day in the morning.
The daily dosage is then increased every 3 days until it reaches 200 mg taken in four doses of 50 mg each.
After the phase-in period, Tramadol may be taken in doses of 50 to 100 mg every 4 to 6 hours, depending on the severity of pain.
If rapid pain relief is needed, the phase-in steps can be skipped, though side effects will be more likely.
The maximum dosage under any circumstances is 400 mg a day (300 mg for those over age 75).
For people with kidney problems, the usual starting dose is 50 to 100 mg every 12 hours; and the maximum per day is 200 mg.
For those with cirrhosis, the usual dose is 50 mg every 12 hours.
Children
Safety and effectiveness in children under 16 years of age have not been established.
